Trained in both adult and pediatric medicine, Dr. Mikail is committed to making this process as humane as possible for you and your family, while providing top quality, personalized medical care at the same time.


Dr. Mikail is a designated Civil Surgeon with the United States Citizenship and Immigration Service (USCIS). Only doctors on the USCIS Civil Surgeon list are authorized to perform immigration physicals. 


The Immigration Physical involves several required steps:

--A comprehensive medical history and physical exam

--Screening for specific physical and mental disorders

--Review of your immunization record 

--Adults age 19-59 typically require the following vaccines: Td/Tdap, MMR, Varivax, and flu (October through March only). Requirements for children and older adults are based on the ACIP/CDC vaccine recommendations. There may be exceptions based on USCIS regulations or particular medical conditions.

--If you are missing required vaccines (or don't have your vaccination record), we will test your immunity through blood tests (titers) and/or order the appropriate immunizations. We prefer not to administer vaccinations you don't need.

--Required blood test for all patients age 2 to 14: Quantiferon-TB-Gold (IGRA).

--Required blood test for all patients age 15 & up: RPR & Quantiferon-TB-Gold (IGRA).

--We do not use the old-fashioned TB Skin Test (PPD), which is less expensive but involves injecting tuberculin protein under your skin, may cause redness and swelling in your arm, and may cause a false positive result if you have had BCG (TB) vaccine.

--Chest X-Ray, if your TB test is positive

--Additional testing, referral to specialists, and treatment if necessary

--Completion of the Medical Clearance Form (I-693), provided by our office


At our office, you will also receive at no extra charge:

--A free, comprehensive Health Risk Assessment (HRA) and analysis

--Personalized health promotion/disease prevention counseling

--A guide to the many federal, state, and local medical resources available to you, so you'll continue to maintain a healthy lifestyle for many years to come


Rates:

For patients without U.S.-based health insurance:

--The initial history and physical exam is $99.

--The RPR and Quantiferon-TB-Gold together are $65 (discounted rate by the lab). Discounts also apply for any additional required lab tests.

--If X-rays/imaging, vaccines, referrals, or treatment are needed for USCIS clearance, these are charged separately. The L.A. County Health Department may provide some of these services at no or low cost to you. We will help locate facilities available to you.

--When your blood tests and vaccinations are complete, you will meet with Dr. Mikail to review your results and sign and receive copies of the I-693 form. The rate for this visit is $49.


For patients with U.S.-based health insurance:

--All rates will be determined by your insurance plan. 

--Most vaccines and some lab tests may be covered in full as part of preventive care. Check with your insurer.

--Blocks of appointments can be scheduled for families who would like to be examined on the same day.
